I would imagine that tgrimes has probably already done the new patient papers for the Immunologist, and one of the questions certainly is or will be "does he have frequent ear infections?"

It's a sign of a PID but underlying illness goes undiagnosed for so long because doctors are looking for the simplest answer, which is usually right.

But, in my unprofessional opinion, H is one of these zebra patients and there is definitely something else going on here.
