I am really, really bummed/disappointed about the Neil deGrasse Tyson business. Oh gosh, I haven't seen this. I met him before he was "famous" and have always been a huge fan. I'm afraid to look.

I was reading this article about hospital closures in rural communities and how Medicaid expansions are disproportionately helping rural communities to keep hospitals & clinic open. Why aren't we pushing to start a truly nationalized healthcare system in rural communities? With limited access to for-profit health insurance in rural communities (few jobs that offer, low wages that make buying in prohibitively expensive even when it's offered) and the ongoing closures of for-profit hospitals in rural communities because it's simply too expensive to operate there, rural communities have few to no for-profit competitors. Not only would a nationalized hospital system be a tremendous help for those in rural communities, but legislation establishing it would probably meet with less friction from powerful for-profit hospital and insurance lobbies, because these are markets they're not in--and won't go into--anyhow. Why aren't we focusing our efforts here? And why aren't we using this as a way to engage rural voters--white or otherwise--with the Democratic party? So similar to the VA, but not just for veterans? Brilliant

Medicare does “subsidize” these facilities in a way now. So all Medicare hospital payments are the same formula but adjusted based on the area through cost of wages, etc. And there are some payment bonuses for teaching hospitals and hospitals that treat a larger proportion of low income patients (basically big urban hospitals, but some rural hospitals qualify). However, there are two designations that many rural hospitals qualify for - critics access hospitals and sole community hospitals. The designations are based on hospital size and distance from other hospitals. They get cost based + a small percentage reimbursement. So presumably enough to cover their costs and keep the doors open. Clearly, it’s not enough to solve the issue as we are still seeing lots of closures and reductions of services. But I think because the government is already involved to a level, maybe people aren’t thinking of a full government run system. I agree it should be on the table though.

So similar to the VA, but not just for veterans? Brilliant Yes. Although given the issues with the VA, maybe we shouldn't draw comparisons. I would love to fix the VA, too. The VA is a solid system but needs better leadership. I.e. not mar-a-lago members.

@heartbot rural healthcare is something NHS has tried to really focus on for a couple of years . We might be able to look at things the UK has successfully implemented when discussing a government run rural healthcare structure.
