No experience with Tamiflu, but I've heard the same story that your pharmacist is telling you. I think one even came from ggandlee. I'm really sorry the kids are sick. kim22 eek yes that'd def be worth it. J's been kind of fine, so I think we will ride it out unless things change at this point. I think that's what I would do. @margaretyrell used it recently and had a bad experience. That's who it was!

I'm laughing cause my FIL taped a Burger King bag on our mail slot in our house, I assume to keep the draft out. Obviously it looks terrible, but also unsure how he thinks our mail will be delivered? People are funny.

tallb, I know you enjoy Black-ish, and I don't know if you saw the older daughter's spin off, Grown-ish. I watched both episodes last night, and I think it's really good. oh goody!! I meant to watch last night and completely forgot. I'll need to look it up.

We did sleep training for S tonight. Took 40 minutes but I think she’s finally asleep or close to it. Fx tomorrow is shorter. This is what I get for letting her sleep with me when she had a bad cough. Will not do that again.

In regards to the discussions earlier about bedtime/wake up time. I’m absolutely not a morning person. I like my time in the evening to myself but I also like to sleep in. S’s bedtime is 7:30, H’s is 8:30. They are rarely asleep right away, so I’d guess they normally fall asleep 30-60 minutes after they are in bed. S has been waking up at like 6-6:30 which is way too early for me. If they would both sleep till 8 that would be ideal.

J often ends up in our bed when we do it, and we can't blame him. He legit has fallen asleep with his hands covering his ears. I make mh go in for check ins, she sees him and often just lays down when she realizes it isn't me😂. He's only somewhat offended.

Tamiflu has been shown to reduce symptom duration by about 12-18 hours when it is started within the first 48 hours of illness, best when started as early as possible. If fever and cough both began on Monday, then he is outside of that time window and it will offer no benefit. Tamiflu is not a great drug and I am not a big fan. If M or C got the flu, I would probably not treat them with it. It does often have side effects, most notably nausea/vomiting, but also more rarely some neurological side effects such as seizures, hallucinations, etc. The AAP recommends treatment for those at greatest risk of severe complications- children younger than 2 yrs, children with chronic medical conditions (moderate-severe asthma, chronic lung disease, cancer, sickle cell disease, immunocompromised/immunodeficient patients, CF, etc), and those that are hospitalized due to a more severe course of illness. If J is within the 48 hr window of his fever onset, but doesn’t have any chronic issue putting him in a high risk group, then I would not treat him. As for prophylaxis for N- how old is she? If J has been sick for greater than 48 hrs (and so her exposure has been greater than 48 hrs), prophylaxis is going to be of little benefit. The prophylaxis dose is lower than the treatment dose. Obviously, if she has had a longer duration of exposure, her risk of getting sick is higher. If she were to actually get the flu, but is treated at a lower prophylaxis dose than true treatment dose, you then expose her to side effects, don’t properly treat her illness, and increase the risk of potentially developing drug resistance. If she is within the 48 hr exposure window, then go for it. If outside of the window, I would not give prophylaxis. However, I would consider starting true treatment dosing as soon as she becomes febrile, should she get sick, as she is less than 2 yrs and so theoretically in a higher risk group.

While I’m here, I’ll give a quick update. We are moved and sort of settled-ish, but my closet and the playroom are still full of boxes and need a lot of work. The house has been almost fully renovated and there are only a few minor things to finish at this point.
I started my new job on Tuesday, but right now it is just lots of meetings, organizing, and mock patient scenarios, etc. There is so much that goes into opening a brand new hospital! It is exciting but also a bit nerve wracking. The hospital and ER should hopefully open in about a month.
DH is still working at his old job 3 hours away for the next 2-3 months. He has clustered his days to minimize the back and forth, but it means I will be solo parenting for longer stretches while he is away. The last time he was gone (Monday night, the night before my first day of work), M was up all night with gastro. It was quite eventful. He just went back tonight, so hopefully the next 6 days while he is gone are less exciting.
The girls started their new daycare this week (M later than C thanks to the gastroenteritis), and it seems truly amazing. They already love it and their teachers are awesome, so that is a huge relief.
Callie, our boxer died very suddenly and unexpectedly at the end of November, just a few days before her 11th birthday. It was so terrible. I took the girls for a short walk to the neighbor’s house and found her dead, laying in her dog house when we got home. Oscar, the little dog, was at the fence crying and howling. It was heartbreaking. He has been super mopey and has refused to eat dry dog food since then. We can’t decide if he is still grieving or if he is now just refusing dry food because he knows we will give him the good canned food if he holds out long enough. Maybe we will get a new dog in the spring or summer after things settle down and we find our new normal routine.
I hope everyone is doing well! All of the new babies have to be due pretty soon now, right?? I will have to live vicariously through all of you!!

Hey, ggandlee! Can I ask you more about this since you're here? Several months ago, I was diagnosed with Primary Immunodeficiency. I'm doing IV IG treatments currently. Would you think I should take it if I did have the flu (I don't ATM, this is just for future reference). Also, the boys are going to be tested but that hasn't happened just yet. Would you give them Tamiflu although we don't yet know whether they're immunocompromised?

As for the boys, it really wouldn’t change much for me unless they were actually also diagnosed with an immunodeficiency. Anyone can be treated, it is just sort of a risk:benefit analysis of weighing the potential side effects, cost, and potential for creating resistance with semi-limited benefit. If I have an otherwise healthy, “low risk” patient within 48 hrs of symptom onset, then I have this discussion with the family and often give them the opportunity for joint decision making, and if they insist or feel more comfortable with treating, then I treat.

cagoldi dd still nurses 2-3 times a day and if I’m not there those are replaced by bottles. I’m not sure who said it but dd also loves soup and it’s something she can’t really hold in her mouth. About the battle of will part, it’s not about resistance, it’s about discovering they have their own autonomy. That being said, if you feel like something is off, listen to your gut. For the first two years people have told us ds problems would fix itself and not to worry, look where we are now, almost every meal is a struggle.

I’m sorry to hear about your dog ggandlee, that’s so hard. I’m glad everyone seems to be settling in well at the new house/daycare/job. kim22 had her little boy a few weeks ago, @xolastunicorn is up next. We have a few girls that have recently announced they are pregnant too. Busy place around here.

Lots of helpful information and the rare side effects sound scary! J had a 99.5 temp on Tuesday PM and then fever came back Wednesday overnight. So that's 48 hours if that first fever counts even if low. He isn't high risk otherwise. N is 13m old and has been sick with mostly ear infections and one bought if pneumonia, but had tubes now. My nephew got flu b on Xmas afternoon. We head spent Xmas Eve and morning with them. My mom then got a fever Thursday and we stayed at her house too Saturday, but she tested neg for the flu, but I kind of think she had it. So exposure I think has been longer than 48 hours. If she does get sick, I will consider dosing her. Congrats on the move and I hope you get settled soon. I'm so glad the girls love the new school. Also, talk about heartbreaking about your dog😢.
