So I noticed that my insurance drug formulary lists Menopur and Gonal-F as covered drugs with prior authorization, but otherwise my insurance says it does not provide any coverage for ART (down to not covering Clomid). Are there uses for menopur and gonal-f that are not related to fertility treatments? Otherwise, I feel like maybe we can try to get them covered with prior authorization!

Maybe insurance has certain circumstances in which they would cover the drugs for ART if you meet certain requirements for needing help? Dunno.

So, I can only speak to my experience so far. Your insurance might be different. We don't have any IVF coverage from our insurance, but our prescription drug plan is managed separately from our insurance (both provided through H's employer). Our pharmacy was able to get our drug plan to cover some of the meds for this IVF cycle. They did not cover Gonal F, but the rejection letter was phrased in such a way that I think they might cover it if we need a second round. Even without the coverage for the Gonal F, though, what they did cover brought the cost down a ton! If it looks like you have any drug coverage, it's absolutely worth looking into.
