How often has this repeated? I'd honestly talk with a doctor if it's more than 2 times.

Constipation is a huge possibility. Ask for an x-ray. I didn't believe it could happen because R was pooping every day, but then started vomiting randomly on and off for over a week. He was full of poop and no one can explain why. Since we've upped the fiber in his diet and added chia seeds he's been much better. I am giving him miralax once a week or so if it becomes hard for him to go or he skips a day. tj has experience with this too. She recommended the chia seeds.

I agree with Brandie. Constipation would be my first guess. Especially since you say he doesn't go every day, but goes multiple times a day when he does go. That's pretty classic.
We add 1/8-1/4 tsp chia seeds to their yogurt or applesauce every day. We do miralax as needed, especially when one of them complains about a tummy ache.

It's happened 4-5 times since the fall. I never thought of constipation. He doesn't go every day, especially at daycare because he doesn't like the toilets there. But, when goes, it's several times in one day. Like weekends he goes several times. I genuinely think they give him too much milk at daycare. Like, he has cereal with milk in the morning, milk with AM snack (like 2 hours later), milk with lunch and milk with PM snack. Do you think that could be contributing to it? I had to tell them to deny bananas to E at daycare. The girl would eat them with every meal if they were available and she was getting so backed up and having a hard time going. So yes, I think any dairy product could be causing issues in excess. Maybe ask them to limit drinking milk? My daycare wasn't perfect, but it helped a lot. And they just noted it on the allergy sheet to not give them to her. But maybe introduce something with more fiber as well. You may want to ask if they could note what he eats and what he doesn't eat for meals and snacks. Along with the quantity (one serving/two?) I find any worker is pretty understanding if you mention possible GI or constipation issues.

I definitely leaning towards constipation based on all this.
The milk can contribute as well. R is the same with his diet - he eats TONS of fruits and veggies already. Broccoli is one of his favorite foods. But even with going every day, he was still TOTALLY full of poop. Once I gave him miralax for a couple of days, as well as some prunes and fiber bars to eat, his poop volume increased a ton. It was pretty bad for about a week as he was clearing out. Now he's going daily (mostly) and the volume is more than it was before, even with daily poop. So I guess he really did need some help. Looking back, he would often ask me to sit with him or hold his hand while he pooped. Now he doesn't need me to do that most days. If he does, I know he needs a fiber boost.
I've just added the chia to his diet this week hoping to get rid of the miralax, but he's only needed the miralax once a week for the last 2-3 weeks (this all started about 6 or so weeks ago).
I was seriously shocked when the ER doc showed us the X-ray. R doesn't really do any of the typical things they tell you not too - consume excessive dairy, eat a lot of "kid" food, etc. But alas, it still happened.
