Dh has to have surgery in Cleveland in a couple weeks, and we will bringing our 4 year old twins with us. My mom and possibly sister will be there to help occupy them, and I need recs for parks or anything that could offer as a break from the hospital. We will be there for at least a week. I will gladly take any hotel recs as well! The place we stayed in last night was not kid friendly at all.

I would say go check out the OH-IO roll call, but that's huge now.
Botanical Gardens Great Lakes Science Center Edgewater Park/Beach Art museum is free
It will really depend on where you are staying and how far you want to travel to get to places. I have no hotel reqs, sorry.

Will you be at the Clinic or UH? If you are at UH on the main campus, there is a children's museum that would be perfect for 4 year olds! We will be at the clinic.

The science center is pretty good and they have a separate area for the younger kids. I'm not sure if the outdoor water activities are open yet. They also have a giant Lego exhibit right now that my 3 year old had a blast at.

The Children's Museum is closed (temporarily but through the end of 2017).
Not sure of your budget but the Courtyard Mariott on Cornell is nice and has a pool. It's close driving distance to the Clinic, and walking distance for you to all the University Circle activities for the kids. (Art Museum, Botanical Gardens, Natural History) Also walking distance to Little Italy.
There's a playground with splash area a short drive or long-ish walk on Martin Luther King at the entrance to the cultural gardens.
6/14 is the first Wade Oval Wednesday which means food and activities and a free concert on the lawn in the middle of University Circle.
Mitchell's Ice Cream in Ohio City (near the West Side Market) is in an old theatre and the kids can go upstairs and look down into the kitchen to see ice cream being made.
Have someone take them downtown on the train and they can run around Cleveland Public Square.

They're rebuilding not far away, but it isn't scheduled to open until the fall. ETA: I see that has been addressed. Carry on!

I don't have any recs for you, but would it make sense to rent through VRBO or something similar instead of a hotel? More space for the kiddos and a place to eat and unwind in a (possibly) larger space?

Will you have a car? I always recommend the Zoo, although it's probably about a 15 minute drive from where you'll be.

Will you have a car? I always recommend the Zoo, although it's probably about a 15 minute drive from where you'll be. We will have a car, and the kids would love to go to a zoo. They have only been to a zoo once before, and I think that the Cleveland zoo is much bigger than the one we went to.

There are a lot of good suggestions already, but I'll add one more!
Cleveland metroparks is a collection of park space through out the area and they host a number of activities that might be fun, but there are also hiking trails and outdoor space to run around. They do cover a lot of different parks so some may be more of a drive than others. The zoo is part of this network as well.

Prestons hope is awesome! We go there once a week in good weather.
I'd also recommend the zoo, botanical gardens, natural history museum and science center. There are metro parks are all around the Cleveland area with play grounds, water features and paths.
There's also Memphis kiddie park not far from downtown that's fun for kids your ages. DS loved it when he was younger.

And definitely check out the zoo - if the weather isn't great you can still go to the Rainforest there, which is pretty cool.

I'm sorry your husband will be undergoing surgery. I hope everything goes as smoothly as possible!
I will + whatever to Prestons Hope, however it's sometimes difficult to keep an eye on multiple kids there. I thought I'd mention since you said you have twins.
The other place I don't think was recommended is the Museum of Natural History - there is a great kids area plus dinosaurs & animals. Basically you can just go around University Circle (which is a few miles, if that, from CC) and visit the Museum of Art, Natural History Museum, Botanical Gardens, Wade Oval, etc.
