I stayed at the majestic elegance in DR several years ago and it was good, the food wasn't great but I don't think it ever will be at an AI. It's not in either location you mentioned but I stayed at secrets Maroma beach in Mexico and loved it. Or have you thought about a non-ai? Aruba was an easy trip with great beaches and good restaurants to try out. I also liked ST. Maarten.

If you're not bringing the kids, I am obsessed with Couples Negril. We used to go every year before we had DS. So laid back. Excellent food. Friendly, wonderful staff. Beautiful beach.

I went to punts Cana for a wedding a couple years ago. We also stayed at the Majestic Elegance resort. We loved it!
We didn't think the food was bad. The steakhouse was yummy! We weren't a fan of the seafood place.
You just can't drink the water. Stick with bottled water. Even to brush your teeth. We didn't get sick.
We did an off resort tour of the capital which was cool. Saw some of the history. The town was very dirty. We saw the underground 3 lagoons which were beautiful.

I'm a huge fan of St. Lucia since I lived there for almost a year but since that's not on the table I'd say Jamaica over DR. It's cozier, safer and more of a laid back island vibe. Plus whatever to thinking the food in the DR sucked.

We just got back from Barbados, I would absolutely recommend. We had a great time and the hotel was decently priced and quite nice. If you are set of Jamaica or DR, I would say Jamaica. They are much more tuned to tourism as an integral part of the economy so it ends up being safer and more accommodating to those unfamiliar with local customs.
